What happens after you refer
- We call the family to introduce ourselves.
- Our nurse completes an intake assessment at a visit.
- We confirm authorization with you and schedule rides.
- We let you know the start date.
Who we accept
Adults with Alzheimer’s and other dementias, MS, ALS, chronic illness and physical disabilities, and adults returning home from a hospital, rehab or nursing facility.
Programs and plans
Ohio Medicaid waiver, PASSPORT, MyCare Ohio (Buckeye, CareSource, UnitedHealthcare), Care Coordination, UnitedHealthcare, Wayne County JFS and private pay. VA contract in progress.
