How to get coverage through PASSPORT
PASSPORT is Ohio’s Medicaid waiver that helps older adults stay home, and it can pay for adult day care and rides.
- Call the Area Agency on Aging. In Wayne, Summit and Stark counties, that’s Direction Home Akron Canton at 1-877-770-5558.
- Complete the phone screening. It checks preliminary Medicaid eligibility and care needs.
- Meet your care manager. They build a care plan with you. Ask for Gilcrest by name for adult day and transportation.
- We take it from there. We coordinate authorization, schedule a visit and set up rides.
Medina County residents work with a different Area Agency on Aging. Not sure where to start? Call us and we’ll walk you through it.
